Body Masculinization Surgery

Body masculinization surgery (BMS) is a common and important procedure in the transition process for the transman. When combined with testosterone therapy, this surgery can result in helping the transman achieve a very masculine looking physique. As a result, the transman typically experiences improved self-esteem and confidence.

The transman that transitions after their puberty, has typically experienced the results of estrogen throughout their body. This includes breast development and body fat on their buttocks, hips and thighs. While testosterone therapy can re-distribute some of this fat to areas more associated with males, some transmen opt for surgery to achieve a more masculine look.

Body masculinization surgery is comprised of targeted liposuction to the buttocks, thighs, chest, and hips. By removing fat from around these areas, the surgeon is able to sculpt a trim and masculine body contour. A genetic female carries more fat in the buttocks area and the shape is more like a heart. Through targeted liposuction, the buttocks can be shaped square and the fat can be reduced. This creates a very naturally looking masculine backside.

While all surgeries carry some form of risk, body masculinization surgery is a safe procedure and most transmen are very happy with their results. During the procedure, the surgeon will make tiny incisions in the treatment areas. Since the incisions are very small, scarring will be minimal and possibly invisible over time. A tool called a cannula is then used to remove unwanted fat via vacuum suction. The length of the procedure will vary depending on which areas are being treated. Absorbable sutures may be used to close the incisions, and the patient will wear a compression garment to reduce swelling and assist in the healing process.

While liposuction is a common procedure performed by many plastic surgeons, it is important to find a surgeon experienced with transgender patients. This is not only due to the skills and experience needed, but because they will have a full understanding of the results you are seeking.

BMS is typcially performed on an outpatient basis while the patient is awake, using a local anesthetic. Recovery takes about one week on average. During this time, strenuous exercise should be avoided, and some swelling, bruising, and pain is to be expected. While serious complications are rare, they have occasionally occurred, so it is extremely important to choose a surgeon with expertise in performing BMS. Additionally, trans men who undergo BMS should have realistic expectations and a good understanding of the risks and benefits of this procedure.
